Translation
The amazing Overlord of KryptonSite has deciphered the jumble. KSite's madCoder has posted a good explanation as to the math behind the decryption here. Interestingly (and frustratingly!), the encryption involves that mysterious 28476 that's appeared THREE TIMES now (once in plain sight and hidden twice). Decoded, the jumbled passage reads as follows:

One important thing about this paragraph is that it looks like we have two more Snowflakes: Snowflake #938617 and Snowflake #914071. So, now, we have a whole slew of numbers that have appeared in this Hidden Prophecy: 28476 (Part I), 434690 (Part II), 852501 (Part III), 881885 (Part IV), a hidden 28476 (Part IV), and now 938617, 914071, and another hidden 28476 (Part V). Interpretation
For convenience's sake, I've moved all of the jumbled paragraphs to the end of the Prophecy, separating them from the non-encrypted portions.
Two orphans joined soul to soul.
The third had made them whole.
Now no one can console.
The orphans of "Smallville" are Clark (not that Jonathan and Martha don't do an absolutely bang-up job as adoptive parents!) and Lana (remember? Her parents got squished by a meteor during the meteor shower? You never heard about that? Well, that's probably because she only mentioned it once in passing, not repeatedly until viewers wanted to gouge her eyes out with a rusty Q-tip...!). The third is Evan, the rapidly-aging Freak of the Week from "Ageless." Evan somehow made Lana feel like she had a purpose (no, hon, you DON'T!) and a reason for being or something like that. And Clark liked the kid too. But he essploded, and no one could console Clark and Lana. Except they were over it by the time "Forever" rolled around, so hmm.
The story ends.

Any good fan of The Matrix recognizes that this is what Morpheus says will happen to Neo if he chooses the blue pill. Other references to The Matrix Trilogy are in Part V and Part VI of the Hidden Prophecy.

In Part III, similar words appeared... if you put them on top of each other in the order in which they appeared, you can see the symmetry:

So, the only word that's different (besides the order of the words) between the two mini-poems, if you could call them that, is the word forgetting appearing in Part III's poem being changed to remembering in Part V. Also... It looks like an "Hourglass," hee.

The spaceship that landed in "Commencement" pierced the earth. The contents of this ship, which appears to be made of a material similar to shale, presented two illusions to test Earth's inhabitants. However, these invaders weren't able to withstand Clark, who gets his powers from Earth's yellow sun. Also, Clark gave up his powers in "Arrival" by not returning to Jor-El by sunset...

KSite's babaojay points out that these are lyrics from Seal's "Love's Divine," a song played in the episode "Legacy" during what KSite's Hiptobesquare calls
[T]he most disgusting angst-y Clana in the history of the world.
To which KSite's Arista 07 reacted by saying,

There are eight names here. Wendel, Haley, Chloe, and Delia were all captured and "collected" by the Freak of the Week in "Forever." Pete is, well, Pete-- gone, but not forgotten. As for Liz, Seymour, and Alice, I'm not sure. Seymour might be a nod to Jane Seymour, whose character (Genevieve) was murdered in "Commencement" by Isobel.

Clark said this in "Pilot" when Jonathan asked him if he was okay.

In "Blank," Clark told Chloe he was considering going to the University of Miami (which, interestingly, is where Arthur "A.C." Curry ("Aqua") went to school before going into hiding), Chloe's response to the news was this snarky line.
Premature combustion
Amnesiac Clark saw Lana in the Talon and eyejaculates (Omar, I love you!), setting a lamp on fire. A disgusted Chloe put out the fire and said something like, "Premature combustion... That's one ability I didn't need to know about!"

Jason and Lana had a big fight in "Blank," and Jason yelled this at Lana. Genevieve also said a variation of this line about Lana to Jason in "Forever" (something along the lines of "She has no idea how much you've been protecting her...").
I plan on leaving a different kind of legacy
Lex said this to Clark in "Ageless" when they were talking about fatherhood. Lex told Clark he thinks Clark will be a good father, and Clark said the same about Lex. However, Lex disagreed and said he plans on leaving a different kind of legacy. And then they moved to Vermont, formed a civil union, and adopted a lovechild. HoYAY!
We didn't find him; he found us
Ahhh, Mama Kent. After finding li'l Clarkie in the cornfield after the meteor shower, Jonathan wanted to turn him in. How would he and Martha explain a new baby? "We found him in a cornfield?" Martha, ever the font of wisdom, replied dramatically... "We didn't find him; he found us!" I love that part of "Pilot." <*sniff*> This general idea iswas repeated in "Ageless," except Evan was the foundling and Clark and Lana were the doting "parents."

This was FINALLY solved by JPMiller over at Rich's World. A lot of us had figured those strange bracketed letters were computer code, and everyone figured out that 04/27/05 was the airdate for "Blank." Apparently, it's a reboot command in some sort of UNIX manual.
The generic command to cause a reboot at a given time would be [ -l ][ -n ][ -q ] [ -t mmddHHMM [ yr ] ] where mm, dd, HH, MM, and yr (optional; it's only necessary if the reboot won't take place the year the code is written) are the two-digit forms of month, day, hour and minute (in 24-hour time), and year, respectively.
Therefore, when Ezra wrote this cryptic passage, he was really typing UNIX code... He predicted Clark's memory would be rebooted at
